(Application) Skills Days 2026 –Supporting a Meaningful Death in Long-Term Care. May 2026.

Perley Health, in collaboration with the Champlain Hospice Palliative Care Program (CHPCP), is proud to present Skills Days 2026 – Supporting a Meaningful Death in Long-Term Care (LTC). This hands-on training day, led by Perley Health and the Perley Health Centre of Excellence in Frailty-Informed Care™, will equip nurses working in long-term care with specialized skills for supporting end-of-life care for residents.

· This one-day training program will be hosted on May 20, 2026, at Perley Health.
· This training day will be repeated on May 27, 2026, at Perley Health.

This education is made possible through a new partnership between the Perley Health Centre of Excellence in Frailty-Informed Care™ and the Champlain Hospice Palliative Care Program.

Through support from the CHPCP and Perley Health, this event is free to nurses working in long-term care in the Champlain region.

The Champlain region covers the following areas:

1. City of Ottawa: The nation's capital and its surrounding suburbs.
2. County of Renfrew: Including communities such as Pembroke and Renfrew.ontariohealthathome.ca
3. United Counties of Prescott and Russell: Covering towns like Hawkesbury and Rockland.
4. United Counties of Stormont, Dundas, and Glengarry: Encompassing cities and towns such as Cornwall, Alexandria, and Winchester.
5. Part of Lanark County: Specifically, the northern portion, including areas like Carleton Place and Almonte.
